Ask your client to call us at (480) 819-8630 to set up an appointment with the specialist of your choice.

Prior to your client’s appointment with us, please fax or email the patient’s pertinent recent medical records, including lab work, surgery reports, radiographs, diagnostic test results, and current medications.
